Understanding IUI: the simplest fertility treatment

IUI places washed, concentrated sperm directly into the uterus at the time of ovulation. It shortens the journey the sperm has to make and improves the odds in a natural cycle without any of the complexity of IVF.
Who it suits
IUI needs at least one open fallopian tube and a reasonable sperm count. It is commonly offered for mild male factor problems, cervical factors, ovulation problems treated with medication, and unexplained infertility of short duration.
What happens in a cycle
The cycle is tracked with ultrasound from around day nine to find when the follicle is mature. A trigger injection may be given to time ovulation precisely. On the day of the procedure the semen sample is prepared in the laboratory, and the insemination itself takes a few minutes with no anaesthesia and little discomfort. Normal activity resumes the same day.
How many cycles are reasonable
Most of the benefit from IUI comes in the first three to four cycles. If it has not worked by then, repeating it further is usually less useful than re-examining the diagnosis and discussing IVF.
