When should a couple consider IVF?

Couples often arrive asking directly for IVF, sometimes after years of unstructured treatment elsewhere. IVF is a powerful treatment, but it is one option among several, and it works best when it is chosen for the right reason.

First, a proper evaluation of both partners

Before any treatment, both partners need testing. For the woman that means hormone profiles, an ultrasound, and an assessment of whether the tubes are open. For the man it means a semen analysis — quick, inexpensive, and too often skipped.

A surprising number of couples who were told they needed IVF turn out to have a treatable problem that responds to something simpler.

When IVF is the right choice

IVF becomes the appropriate next step in defined situations rather than after a fixed number of failed attempts at everything else.

  • Both fallopian tubes are blocked or damaged
  • Severe male factor infertility, often needing ICSI
  • Several IUI cycles have not worked
  • Advanced maternal age with reduced ovarian reserve
  • Significant endometriosis
  • Unexplained infertility of long duration

What a cycle involves

About ten to twelve days of injections to grow several eggs, monitored by ultrasound. Egg retrieval under short anaesthesia as a day-care procedure. Fertilisation in the laboratory, embryo culture for three to five days, and then transfer of a single embryo. A pregnancy test about two weeks later.

An honest word about success

No clinic can guarantee a pregnancy, and any that does should be treated with caution. Outcomes depend heavily on the woman's age, the diagnosis and embryo quality. We tell couples what their own reports suggest, and we say plainly when the odds are low.
